As the LTE resource block is 180KHz wide and it is the minimum allocation in terms of bandwidth, UE power spectral density will be small compared to a say GSM terminal because uplink max power is only 22-23 dBm compared to a GSM UE that can transmit at 27-28 dBm that can potentially lead to coverage gaps in overlay scenario.
What are possible solutions ? :?:

I think there are two issues with voice over LTE.
First is about hooks needed in core for 911 etc . Whole VOLGA setup is there to workout and comeup with a workable solution.
Second is a fundamental RF issue as you suggested. Basically fundamental issue is LTE is not efficient for small persistant bandwidth required services such as voice. But it is not too difficult to resolve. Solution is to use voice bundling. So basically you can combine two voice packets in one and send it together in one frame.
